The purpose of the present study was to determine the effects of a lifestyle intervention on the incidence of vascular complications in patients with established diabetes. This Japanese randomized controlled study included 2,033 men and women aged 40-70 years at baseline from whom 1,304 participants were analyzed for occurence of vascular complications after 8 years. This study reported a significant but small contribution of the lifestyle intervention to the improvement in glycemic control. The results also showed a significant reduction in the risk of stroke among the patients in the intervention group compared to the control group independently of known classical risk factors. These results suggest that more closely supervised lifestyle modification intervention programs should be considered in patients with long-term diabetes.
