In this perspective paper on glycemic control in patients with type 2 diabetes, the review and a critique of recent large randomized trials suggests that tight glycemic control burdens patients with complex treatment strategies, hypoglycemia, weight gain and costs without providing clear benefits. Montori and Fernandez-Balsells believe that clinicians should prioritize supporting well-being and healthy lifestyles, preventive care and reduction in cardiovascular disease in patients with type 2 diabetes. They recommend a mutual dialogue between the patient and the clinician in order to set the HbA1c target. Furthermore, how to achieve this target should also be discussed so that the process will reflect the patient’s personal and clinical context as well as their informed values and preferences.
