This paper looked at the association between apo B/apo A-I ratio and insulin resistance assessed by homeostatic model assessment (HOMA2) in individuals who participated in the Third National Health and Examination Nutrition Survey (NHANES). The authors found that the apo B/apo A-I ratio is associated with insulin resistance independent of the clinical criteria of the metabolic syndrome. These results suggest that the apo B/apo A-I ratio could be an index of the dyslipidemic profile that would probably capture more information in insulin resistant subjects than the clinical criteria of the metabolic syndrome. This paper was accompanied by an interesting editorial written by one of the pioneers of the use of apo B in clinical practice for diagnosis of atherogenic dyslipidemia and the evaluation of therapy.
