This editorial by Dr. Luis M. Ruilope introduces papers published in a Journal of Hypertension supplement. In this supplement issue, Drs. Volpe and Tocci discussed the concept of global cardiovascular risk presented in the Guidelines of the European Society of Hypertension/European Society of Cardiology. Drs. Waeber, de la Sierra and Ruilope reviewed the next stage in the evolution of the disease which is characterized by a detection and treatment of target organ damage. Dr. Williams analyzed the different treatment strategies presented in the European Society of Hypertension/European Society of Cardiology Guidelines. Finally, Drs. de la Sierra, Zamorano and Ruilope presented data of a survey of Spanish physicians that sought to identify to what extent the European Society of Hypertension/European Society of Cardiology Guidelines are implemented in Spain and what are the expectations of the "front-line" physicians regarding these Guidelines.
